Referrals

Getting timely support to children and young people who have been bereaved is vitally important.

 Here you will find all the information you need to know to refer a child or young person who has experienced the death of a significant loved one and who may need further support from Sandy Bear Children’s Bereavement Charity.

 Before you start, you need to download our referral form and complete the information requested. This includes having consent from the family of the child or young person you wish to refer into our service.

 Completed forms can either be emailed to referrals@sandybear.co.uk or sent to the address contained at the end of the referral. Please note that we do not accept referrals without a referral form.

 You (the referrer) will be contacted to acknowledge receipt of the referral. A bereavement practitioner will make direct contact with the family to explain the next steps in receiving appropriate support in line with the needs of the child or young person.

 As a referrer, you will be contacted again when the child or young person has been discharged from the service after receiving appropriate support.
